2: JAMB Original Result Slip showing the Candidate’s passport Photograph.

3. F.U.O Post-UTME/Supplementary Slip.

4. SSCE/GCE/NECO/NABTEB Certificate(s).

5. Birth Certificate or Statutory Age Declaration.

6. Local Government Area Identification Letter.
  
7. Twenty four  (24) Colour Passport Photographs.

STEP (1): ICT/MEDICAL: The first step is to make payment for Acceptance and Medical Fee which is #38,000 excluding Ict and pocessing fee 

b: Signup on the school portal and update your biodata carefully

c : Download and print the following document from the Fuotuoke school portal

2. Acceptance Form
3. Biodata Form 
4. Guarantor's Form
5. statutory Declaration Form
6. Matriculation Oath Form
7. Medical Examination Schedule
8. Medical Report Form

d: Visit Fuotuoke Medical Centre on Schedule Date for Physical Examination

B: Faculty Office: Freshers are expected to visit their Faculty office with the  following documents as they would be needed for their Faculty Clearance

11. Printed Biodata 
12. Printed and Signed Copies of Guarantor's Form'
13. Printed and signed copies of Statutory Declaration Form

N.B: Faculty Clearance Charges Differs 

C: Activation For Payment of the Following :

1. School Fees: You are expected to generate RRR from the school portal to pay your School Fees
2. Hostel Fee: You are Expected to Pay #10,000 for School Hostel
3. Student Passbook fee: 
4. Matriculation Gown Fee: This ranges from #1000-#3000 depending on your faculty

STEP (3): BURSARY: This is the last step of registration for freshers to become a fully registered student of Federal University Otuoke,Bayelsa, take note of the following

A: Visit your Faculty Account Office 

1. Obtain your recipts for all payment
2. Obtain your passbook

B: Proceed to FUO ICT for the following
